You are warmly invited to attend a free NHS Health Check at the Practice. NHS Health Checks are being offered to people aged between 40 and 74; the check is to assess your future risk of developing heart disease, stroke, kidney disease or diabetes. If there are any warning signs by taking early action, we can work together to improve your health and prevent the onset of these conditions.

The check consists of two appointments:

• The first appointment will be to ask you some straight forward questions such as age, sex, family history, height, weight, and measure your blood pressure. There will also be some blood drawn to measure your cholesterol and glucose levels and your kidney function. To book this please refer to the text message we have sent to you and click on the link at the bottom of the text to book the blood test with any one of our HCAs. Your second appointment will be booked by the HCA after you have had your first appointment
• The second appointment will be a 15-minute telephone appointment approximately a week later, when the results of your blood test will be available, and we can discuss the results with you.
