What is the role of a Health and Wellbeing Coach?

The Health and Wellbeing Coach is a non-clinical health professional, employed by Hadwen Health, who supports people to take proactive steps to improve the way they manage their own health based on what matters to them. Each session provides a space for the patient to reflect on goals they would like to achieve and consider barriers which could get in the way of this happening. Follow up sessions are offered at approximately 2 weekly intervals to provide an opportunity to touch base.

Who can access this service?

This service is open to Hadwen Health patients over the age of 18 years of age. Individuals can self-refer to the service or a referral can be made via a health professional or patient services.

There are no fixed criteria for a referral for Health Coaching, but the following may be of help:

  • Patients living with any long-term health condition.
  • Patients wishing to lose weight, be more active, reduce stress, or balance competing demands of life.
  • Patients experiencing low self-esteem or low levels of anxiety/depression.
  • Patients struggling with prioritising health goals while caring for someone.

What can I expect after a referral is made?

You will receive a phone call where the Health and Wellbeing Coach will gather some information about yourself and your lifestyle. Following this conversation, if it is thought that you will benefit from health coaching, you are likely to be offered a 45 minute face to face appointment at the surgery.
