Patient Consent & Terms of Agreement the Social Prescribing Service

Please be aware that the Social Prescribing Service is free to you (the patient), but does cost money to run and has a high demand of people waiting to access it. It is therefore important that you are committed to attend all appointments where possible.

What you can expect from your Social Prescribing team:

We will aim to:

  • Help you to set goals that are realistic and achievable
  • Listen to you and work to support you in identifying and achieving your goals
  • Help you write a plan of how you will achieve your goals and help you make the right connections
  • Recommend groups or activities that may be helpful to you in achieving your goals
  • Look after your information and only share it with those you have given us permission to do so
  • Monitor your progress so that we can help you make progress

What the social prescribing team will expect from you:

  • That you try to keep appointments, or give us as much notice as possible if you need to cancel.
  • Give new things a go and/or try them for a while
  • Tell us honestly about how you feel things are going

Please be aware that failing to attend an appointment without notifying us may result in you being marked down as a “Did Not Attend” (DNA) for that appointment. Having two or more DNAs may result in you being signed off from the service. Patients who are signed off may still be able to access the service at a later date, but may have to wait up to 6 months to re-engage.
